Telehealth for UTIs

One of my favorite projects while working at Healthlabs was for UTItreatment.com. This startup provides telehealth options for women dealing with a urinary tract infection. Using this service, users fill out a questionnaire about their UTI symptoms. A doctor then contacts the user to review their diagnosis and calls in a prescription to the user's local pharmacy.
The Project
For UTItreatment.com I was tasked with redesigning the main questionnaire users take when using the service. Our research on the site found that many users abandoned the questionnaire mid way through. Our hypothesis was that the current design was boring and a hassle to get through as it had multiple pages with lots of questions per page.

THE Plan
I worked with the marketing department to come up with a design similar to how takecareof.com does their questionnaire. Our idea, like takecarof, was to keep the user engaged with pleasant UI and put one question per page rather than multiple. To wireframe our plans, I used sketch to draft ideas and I made custom icons using Illustrator. To prototype it, I used UXpin.com to make cool UI interactions giving the prototype the UI feel we were looking for. After working through a few iterations with our CEO, our design was approved. Currently the design update is still being built but I am very excited for the end result. Here are a few screen shots of the questionnaire:
